Please arrive 15 minutes early for your first visit. When you arrive, check in at the front desk and provide any completed paperwork and/or physician referrals. If you were unable to complete the new patient forms in advance, you will be asked to fill them out at that time.
You will then meet with your physical therapist, who will perform a comprehensive evaluation. This includes a review of your medical history, as well as an assessment of strength, joint mobility, and functional movement. Based on this evaluation, your physical therapist will develop a personalized treatment plan tailored to your specific needs and goals.

Visits are scheduled as 40–55 minute, one-on-one sessions with a physical therapist or physical therapist assistant. Your therapist may ask you to arrive early to warm up prior to your session to help maximize your time during treatment.

Every session is 1-on-1. No double booking. Just focused time to coach, correct, and make sure you’re moving and exercising the right way.

Please wear or bring loose, comfortable clothing (such as a T-shirt, shorts, and athletic shoes) to allow for proper evaluation and movement of the affected area.
For upper body concerns (neck, upper back, or shoulders), a tank top or loose-fitting shirt is recommended. For lower body concerns (back, hips, knees, or feet), shorts or loose-fitting pants are best.

For your first visit, please bring a photo ID, your insurance card, a physician referral (if applicable), a list of current medications, a list of past surgeries, and any completed paperwork (or plan to arrive early to fill it out).
If you have operative reports, MRI, CT scan, or X-ray results, please bring those as well. Additionally, if you use any assistive devices—such as a splint, brace, crutches, or a walker—be sure to bring them with you.

Visit frequency typically ranges from 1–3 times per week. The exact schedule will depend on your individual needs and will be determined after your initial evaluation with your physical therapist. Our front desk team will assist you in scheduling your appointments before you leave.

Prior to your first visit, our insurance coordinator will contact your insurance provider to verify your coverage and any requirements. We will review this information with you before you begin your physical therapy treatment. Private pay options are also available.
